Sign in Open the menu WorkspacePredictive Care
On a phone, tap the menu icon at the top left. Expand the section above, then select the tool. On a computer, use the left sidebar.
How to use it
- 01
Choose a patient and review the latest SOAP note loaded into the form.

Add relevant ICD-10 context, then select Assess 30-day risk.
- 03
Review each score, its drivers and the suggested action against the patient's current condition.
- 04
Record new vital readings and provide feedback on the assessment.
